Framework has a rare and exciting opportunity for a role in Social Housing Related Support
working Mondays to Fridays and getting out to support people in the Community. The
Support Planner will join a small and friendly team based in Mansfield.
The Team works closely to provide support to vulnerable individuals and support to each
other, dealing with a variety of needs/ issues and looking at practical ways of delivering an
effective service with positive outcomes.
**Duties & Responsibilities**
- To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
- You will have caseloads with Tenant’s in properties across the county (mileage system for car drivers provided).
- Prepare Support plans.
- Work closely with Frameworks Housing Management team.
- Assist service users with basic budgeting and life skills in order to support them to move on to live independently.
- To liaise appropriately with Key partners, stakeholders and service providers to ensure appropriate support is offered to all Service Users.
**Experience Needed**
- Some Support Worker experience or similar
- Knowledge of Tenancy Management
- Knowledge of Support plans and risk assessments
- Must be able to drive and have own transport
