**COMMUNICATION AND RELATIONSHIPS**: Collaborative working with clinical teams both with and outside of the trust to ensure the appropriate management of patient information and coordination of patient pathways. Work with trust staff to ensure all cancer patients have a booked first appointment/investigation and establish robust processes with all involved in the cancer patient pathways to improve the coordination of the patients journey e.g. radiology department. Maintain confidentiality and manage information sensitively.

Demonstrate discretion and respect when communicating with patients, colleagues and other agencies. Co-ordinate the flow of patients through the colorectal department ensuring a high standard of care and communication at all times, demonstrating regard for the patients needs. Communicate effectively with all members of the Multidisciplinary Team. Supporting and promoting teamwork generally through effective written and verbal communication.

Maintain close liaison within a multi-disciplinary teams in primary, secondary and tertiary care referral environments. Establish and maintain productive working relationships within the teams and all other internal and external agencies as required. Deal with large volumes of information including co-ordinating examinations for patients with complex needs. Provide a contact point for patients, next-of-kin, radiologists, Consultant and Oncology Staff, referrers, wards, porters and other stakeholders, and act in a manner ensuring privacy and dignity for all patients.

Prioritise patients for booking, identify the requirements of the individual and ensure everything is in place in order to guarantee a smooth patient journey and optimise capacity. Facilitate and co-ordinate the function of the multidisciplinary team meetings by ensuring a list of patients to be discussed and other relevant information is prepared and distributed in advance of the meeting. Contribute to the monitoring and analysis of throughput, capacity and demand and be able to identify and implement methods of improving the service. Maintain database of cancer surveillance patients, ensuring investigations are performed at the appropriate timescales.

Provide information for national databases i.e NELA. ANALYTICAL AND JUDGEMENTAL Use your own initiative to problem solve and find acceptable solutions for all parties. Understand the cancer targets for LGI including the 2 WW, 28 day and 62-day cancer pathway in accordance with trust and national policy. Keep abreast of any local, regional and national changes to cancer targets that may affect the service.

Be aware of, and participate in, risk assessment for the benefit of self, colleagues and clients, in line with the Risk Management Policy and Health & Safety at Work Act 1974. Prioritising own workload to meet deadlines and demands of the service. Ensure the maintenance of accurate and efficient record keeping, including both paper and computerised systems. To make decisions regarding the prioritisation of patients.

To run statistical analysis of data to track performance and KPIs. PLANNING AND ORGANISATION: Understand the processes involved with performing scans/tests and utilise this knowledge to organise lists to optimise both patient experience and capacity. To initiate and act as facilitator to expedite urgent referrals. To help accelerate patient flow by maintaining an awareness of potential delay points and escalating/managing issues: vetting, blood results, portering etc.

Identifying how service can be improved for patients and staff. Take responsibility for own personal development to meet current and emerging role requirements. Identify the need for, and chase and obtain missing information and/or documentation required to enable scans. Identify opportunities for process improvement and use initiative to lead the implementation of these ideas.
