Social Prescribing Link Worker

Due to expanding our service, LCVS are looking for a Mental Health Social Prescribing Link Workers to join our team. Link Workers are part of a multi-disciplinary team within the Integrated Place Based team and the wider neighbourhood team.
Area & Hours roles available:
Spalding 2 x 30 hours, South Lincolnshire Rural 1 x 37 hours & Stamford & Bourne 1 x 15-22.5 hours
We are looking for organised, self-motivated, and confident individuals to join our established Social Prescribing teams working with adults with complex health and mental health needs helping them to access the support they need to live independent and healthier lives.
Applicants must have experience of working with adults and have a background in health and/or social care or experience of supporting people’s wellbeing in a community setting.
You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing social and emotional challenges as well as have great interpersonal skills and ability to support people, community groups and local organisations.
You will work alongside multiple mental health services and GP practices within that locality, independently managing a caseload and new referrals, providing personalised support, and connecting participants with community support and services as well as health care providers to improve independent living, health and wellbeing.
It is essential that you have an understanding of how to prioritise need, be able to recognise when to refer on to specialist services, being confident in your ability to communicate at different levels with people from all backgrounds and across both the health and social care sectors.
You will need to demonstrate a robust knowledge of confidentiality, data protection, safeguarding and boundaries in all aspects of your work. Car driver essential.
