**Salary: £9.90 per hour + £1 service level enhancement due to complexity of support**

**About the Role**

The role of the Support Worker is an opportunity for a pro-active person to join an evolving Care Company. We are looking for a dedicated and empathic person.

This project in particular will be supporting a young man who has transitioned from hospital to a supported living environment. He will require a staff team who have experience in supporting people with complex behaviour to reach their potential. They will support with all aspects of independent living, managing a tenancy, budgeting and day to day living as well as supporting this gent to settle into his new home.
- Demonstrate positive, proactive and organised people skills, that will support and develop the skills of the people we support.
- Take the lead to help the people we support to understand their responsibilities and by doing so ensure that safe, effective, caring, responsive and well lead support and care is delivered at all times.
- Be observant and able to do dynamic risk assessments in an environment.
- Support the Managers & Team Leaders in managing transitions into and away from the service. You will be able to liaise professionally with all outside professional agencies, and ensure that all information is shared appropriately.
- Ensure that all the people we support have person centred support as per their care and support plans. You will be able to follow and review these plans as per any changes that are required.
- Empower the people we support to be actively involved in their own goal setting.
- To oversee and support the development of excellent quality within the service. You will maintain excellent service user records. Working in a supportive manner that empowers the service user to live their life in their chosen manner.
- Have experience of completing both written and electronic notes.
- To fully understand the principles of 'safeguarding vulnerable adults'.
- You will advocate for and protect the best interests of the individuals within the boundaries of the Mental Capacity Act.
- Due to the service being in operation all 24 hours, the shifts include days and nights.
- Having a driving license is desirable.

We very much welcome previous experience working within a supported living environment, the staff team will consist of established team leaders and a dedicated deputy manager & Manager oversight.
