Pay increase (25p per hour) after successful completion of probationary period (six months).
An exciting opportunity has arisen for a motivated and patient-centred Cancer Care Co-ordinator to work at Sutton Coldfield Group Practice.
This post will involve working closely with the partnership Cancer Care Lead to provide a holistic and single point of support for patients living with cancer. The successful applicant will need to have strong administration skills and be able to proactively engage with patients, clinicians, and external agencies.
This is a new role for SCGP and as such has scope and opportunity to offer a position instrumental in offering something innovative to our patients. Internal process development and external local knowledge will be imperative to the success of the role. We are looking for a strong administrator, ideally with experience of working in health care and who can take a proactive approach to supportive services for our oncology patients.

**1. Job Role & Purpose**
The PCN Cancer Care Co-ordinator provides essential support to the delivery of the cancer care services for our Primary Care Network (PCN).
The purpose of the role is to effectively process and manage the administration of our cancer care services and signposting, plus GSF record maintenance, coordination of meetings, taking meeting minutes and managing actions.
The post holder will liaise with the Partnership Cancer Care Lead on progress and potential process improvements.
**2. Key Duties & Responsibilities**
- Undertake effective, efficient, accurate and on-time processing data, including read coding and GSF list updating.
- Prepare reports accurately, completely, comprehensively and on time for GP review.
- Coordinate clinician participation in and preparation of GSF meetings
- Maintain accurate records.
- Undertake effective administration activities including scanning, copying, taking meeting minutes and monitoring inboxes across our clinical systems.
- Undertake effective and appropriate communications to staff, patients and partners as directed in relation to administration and care. (Internally and externally)
- Provide support to patients during cancer care reviews, to include signposting to external agencies.
- Ensure that alerts on patient records are in place and maintained.
- Liaise with the organisations Cancer Care Lead to identify, recommend and implement process improvements as required
- Ensure that reporting and systems are in place so that management is always fully appraised.
- Ensure that information relating to patients, colleagues, other healthcare workers or the business of the Partnership is only divulged to authorised persons in accordance with the Partnership policies and procedures relating to confidentiality and the protection of personal and sensitive data.
- Have a working knowledge of all systems used within the Partnership that support the clinical functions
- Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team’s performance
- Work effectively with team members, colleagues and other relevant stakeholders to
- meet patients’ needs
- Effectively manage own time, workload and resources
- Assist with, record, and monitor the delivery of GSF training to all SCGP staff if required.
- Maintain the protocols that detail the activities performed in the delivery of the role,
presenting to line manager for approval of any key changes
**3. Performance Standards**
- Establish and maintain effective communication pathways with practice staff and external teams appropriate to the role
- Ensure effective and efficient administration activities are delivered on time, accurately and comprehensively at all times.

- Continue to learn independently and to develop professionally, keeping up to date with requirements and regulations contractually and procedurally.
- Ensure clinical involvement in Cancer Care function is kept to expected level.
