The post holder will closely with the Transformation Director, LTC Clinical Director and Head of Operations to continue our service development journey. The post holder is responsible for designing and initial delivery of the key transformation objectives for long-term conditions and community diagnostics, including other related transformation areas. The post-holder will lead on the design and delivery of a design and development pipeline for new ideas and projects to work through. Ensuring that all relevant internal and external stakeholders are involved in this process and relevant progress is communicated to these stakeholders.

The post-holder will work closely with the Head of Operations and Deputy Head of Operations to move those successful pilots from the pipeline process into actual ‘business as usual’ operational delivery. They will administer clinical governance processes within their areas and assist in the management of risk. The role will periodically be required to support the migration of test/pilot services into core services. A key part of the role with be working with the BSol ICS (ICB and ICP) to ensure ongoing success of transformation work. This includes the development of business cases and underpinning impact data to ensure we support the BSol system in its improvement and transformation objectives.
**Key responsibilities**
Managerial
- Manage staff including clinical, administrative, information and managerial groups, together with delegated responsibility.
- Provide day-to-day support to pilot services as required until migrated into operational delivery.
- Responsible for budget setting process for defined service areas, working closely with appropriate clinical and non-clinical staff, and reaching agreement on staff establishments. Plan for the reconfiguration of services, which enables them to be delivered in a manner which offers better value for money.
- Contribute to the production of service development proposals for the defined service areas, including costing and income projections.
- Responsible for the delivery of transformation pilot KPIs.

Responsible for the implementation of new projects and initiatives following detailed analysis and interpretation of information to inform decisions which improve services delivered to Patients and the operational effectiveness of defined service areas.
- Responsible for developing strategic and business plans for the defined service areas working closely with the Director of Transformation, clinicians and non-clinicians as appropriate.
- Develop working partnerships with key stakeholders within the local health economy, representing the organisation.
- Contribute to the formulation and execution of pilot service reviews and recommend and implement different models of care as appropriate, involving clinical and managerial colleagues from the organisation.
- Utilise information flows that support the monitoring and improvement of clinical services within defined areas.
- Ensure effective communication systems for all staff are in place enabling local and corporate information to be shared with staff within the defined service area for views to be fed upwards.
- Where required to deputise for Transformation Director and cross cover Head of Operations if required.

Clinical Governance and Service Standards

1. Work in conjunction with clinical governance lead to provide information and actively contribute to the clinical governance agenda.

2. Ensure patient involvement in the planning of services, enabling care to be patient centred.

3. Oversee pilot services in accordance with relevant national clinical standards NICE, NSF’s, Cancer Plan, etc. and local clinical governance requirements.

4. Ensure clinical advice and support is available at all times to staff within the defined service areas.

5. Responsible for monitoring, feedback and evaluation systems for pilot services to assure the quality of services provided to Patients.

6. Participate in the development, production and implementation of policies and procedures that facilitate the operational and quality effectiveness of the defined pilot service areas.

7. Implement relevant data collections and feedback mechanisms that improve services delivered to Patients and the operational effectiveness of the defined service areas.
