It's a great time to be a part of the Community Mental Health Team in Medway as we are developing the Clinical Care Pathways leading to an improved and innovative service for our Service Users, Carers and Staff.
The role of an Active Review Clinician is to monitor and review patients 4 weekly and ensure safe support for patients who are awaiting pathway interventions. Active Review consists of a band 6 clinician, band 5 and a band 3.
If you are interested in becoming a lead health care professional and you are focused on promoting recovery for those people diagnosed with severe and enduring mental ill health. We provide comprehensive multi-disciplinary services and work closely with local specialist mental health teams, tertiary service providers, primary care, local statutory and non-statutory agencies, service users and carers.
We are looking for motivated and enthusiastic mental health practitioner and who has a strong sense of initiative and an excellent team working ability. Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high level interventions & community assessment skills.

The Senior Mental Health Nurse is a key clinical leader within the multidisciplinary care team and will have continuing responsibility to assess, plan, coordinate, deliver and evaluate care, communicate findings, influence and drive change and promote health and best practice, patient safety and experience.
The Mental Health Nurse will be professionally accountable to the Executive Director of Nursing, and operationally accountable to the Matron / Clinical Service Manager.
**KEY RESULT AREAS**:
- Carry out and coordinate comprehensive, systematic nursing assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measurement.
- Work collaboratively with other disciplines, service user and their carers to agree a holistic person centred care plan that addresses the needs identified through assessment.
- To deliver and evaluate safe, person centred care in partnership with the service user and their carers to support recovery.
- To act as a clinical leader supporting best practice in line with NICE guidance.
- To facilitate and review approved Quality Improvement initiatives within the care setting.
