We're seeking a dedicated **Senior** **Hospital Liaison Recovery Coordinator**to join our Peterborough team. The post-holder will be an integral member of a multi-disciplinary team. They will undertake duties involved in the day-to-day operation, management and development of drug and alcohol treatment and will be responsible for setting up pathways and delivering the full range of services in the local Hospitals.

**Full Time Hours**: 37.5 per week

**Full Time Salary**: £32,002.35 - £34,214.20 (based on full time working hours, pro rata for part time)
- Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

**Responsibilities**:
**The Role**:

- To develop integrated working within local hospitals which will reduce the impact of repeat hospital admissions for drug and alcohol users as well as reducing mortality rates by setting up robust discharge plans for service users being discharged from hospital into the community.
- Utilise a range of evidence-based assessment tools to assess levels of problematic alcohol use and develop recovery plans based on suitability for community or inpatient detox.
- Developing therapeutic alliances with people who have substance misuse problems within a hospital setting and developing effective partnerships with service users to support their individual recovery journeys.
- Ensure the smooth transition of service user’s clinical needs into community care with the service.
- To advise hospital personnel on treatment options for problematic drug/alcohol use and opiate/alcohol detoxification.
- Utilise psychosocial intervention, including ITEP, in both one-to-one and group settings to promote engagement in treatment services and ownership of Recovery Planning.
- Work flexibly within an agreed number of hours. The service will include an ‘out of hours’ component that will be reflective of the peak times for alcohol and/or drug attendance to A&E. Hours of delivery will be reflective of local needs and respond flexibly and will include evening and weekends.

**About You**:

- To thrive in this role, we're looking for someone who has:

- Strong background in delivery of Substance Misuse interventions in an acute hospital setting.
- Have an understanding of substance misuse issues and experience of working within a similar field.
- The ability to learn from mistakes and share learning with the wider team to improve service delivery.
- Excellent communication both written and interpersonally to a variety of stakeholders including Service Users, staff and managers, with the ability to influence in order to improve presenting issues
- Line Manager experience.
- Background in maintaining and developing partnerships working with stakeholders.
