Health and Social Care Co-ordinator

You will be joining a friendly, professional multi-disciplinary team in a busy environment In Blandford, where a flexible and adaptable approach with excellent multi-tasking abilities is required, as well as a good understanding of Microsoft Office especially in Excel.

The Blandford Integrated Community Rehab Team is a multi-disciplinary team that provides rehab assessment and therapy to both the local community around Blandford and also Tarrant Ward within the hospital itself. The team´s goal is to help reduce hospital admission and to facilitate appropriate discharge for any residents within the area.

Services offered include Physiotherapy, Occupational Therapy, Rehab Nurse and access to Speech and Language Therapy. The team can signpost to other community services if appropriate.

This is a demanding position and requires the post holder to be highly organised in managing their workload to efficiently meet the needs of the service

In this role you will receive plenty of support and regular supervision sessions if and when required.

To provide support; administrative leadership and general office management to deliver a high quality, comprehensive care co-ordination service to the integrated Health & Social Care Team.

The role requires a flexible and dynamic person with good organisational skills, excellent Microsoft especially Excel computer literacy and a can-do attitude. Due to the Urgent nature of the work it can at times be pressured and stressful requiring a flexible and adaptive response.

The care co-ordination role within the integrated Health & Social Care team is key to ensuring both urgent and non -urgent referrals are actioned in a timely way. Excellent communication and interpersonal skills are essential as you will liaise with multidisciplinary professionals, GP’s and voluntary organisations.

The post holder will be responsible for their own workload and will also include supporting the ICRT admin team as when required.

The postholder will work their hours over 4 days between the hours of 8.00 - 4.00 and may be will be required to cover a 7 day week service, inclusive of bank holidays.

Weymouth & Portland CMHT is a busy and friendly team who work together promote recovery with people with complex mental health conditions. We are a supportive team who enjoy working in this area. We regularly take students and find this a key part of our role.
