Community Occupation Therapist

Eden Brown Synergy are looking for a Band 6 Occupational Therapist to work in the Nottinghamshire area with a Community Therapy Service.
Applicants must have Community OT experience and be HCPC Registered.
The job role is to provide a specialist OT assessment to people referred to the Community Therapy Service. The OT must be able to work in a variety of community settings carrying out assessment, goal setting, implementing treatment programmes for clientswith complex problems/needs.
Other responsibilities include:
Evaluating treatment programmes in conjunction with other health and social care staff.
To support patients/clients to improve or maintain their level of independence and quality of life.
To be responsible for planning safe transfers of care.
Promote an understanding of relevant risk assessments and appropriate interventions in the wider health & social care community.
You will need access to your own vehicle for the community based role.
