The post holder will work within the Assessment services for Older People to provide a quality responsive service to patients, within the Trust presenting with Mental Health Problems. The post holder will be responsible for and will undertake assessment, planning and interventions and evaluating Mental Health care for people with Mental Health needs accessing the service. Working within the Rapid Access Service, and where required within other clinical areas within older peoples services. Deployment as part of the Rapid Access Service will mean that a high proportion of the work is carried out in peoples homes.

The post holder will promote clinical excellence and provide clinical advice and health promotion with nursing staff and other professionals across the Trust. The service provided will follow the quality principles of appropriateness, equability, accessibility, clinical effectiveness and efficiency. Care given will be of an evidence base and will enforce ethical and professional aspects. This job description is an outline of the main duties of the post.

The postholder will be required to undertake other duties commensurate with the grade as directed. The content of this post will be reviewed in consultation with the postholder when necessary and in line with the service developments. Key Result Areas: 1. To understand his/her own role within Rapid Access Services and within Older Peoples Mental Health Services, and scope of that role, seeking help when unsure.

2. To prioritise his/her own workload, and organise it to meet these priorities. 3. To explain clearly to service users and their carers: Own role, responsibilities and accountability within the Rapid Access Service.

To respect individuals privacy, dignity, wishes and beliefs, minimising any unnecessary discomfort and encouraging full participation in the care process. 5. To obtain: Informed consent to the assessment process. Information on peoples needs and the overall context in which they live.
