Upwards Care Solutions are a 16+ provider with home across the North of England, primarily Greater Manchester, Lancashire, Crewe and Hull. We pride ourselves on providing a supportive and empowering environment for our Young People in order to prepare them for independence.

We are looking for a Referrals Coordinator who will be the main point of contact for external local authority clients. You will be liaising with other members of the team to ensure suitable referrals are made.
- Coordinating all aspects of referrals for the group regarding: enquiries, referral coordination and placements.
- Promoting and nurturing positive relationships between internal and external stakeholders
- Working with the Head of Operations and Senior Service Manager to manage all data arising from the admissions and referrals process: recording and tracking data, analysing data, identifying, and reporting on trends, strategic opportunities, and risks.
- Reviewing details supplied by Service Managers, Registered Managers and local authorities regarding the level of care provision required
- Collating information regarding young people’s backgrounds and referral needs and following processes
- Keep up to date with new legislation, regulations, contracting authority guidelines and communicating the same to all staff.
- Processing large amounts of information accurately and efficiently.
- Reporting regularly on the progress of referrals and placements
- Recording accurate data in the placement team’s data management software system to monitor and track referrals as well as relationships across local authority accounts.
- Participating on the On-Call Rota.
- At least 1 year experience in the care sector
- A high level computer literacy skills with experience in the use and management of databases and reporting.
- A clear understanding of the issues regarding placements and the relevance of those in a commercial and competitive environment.
- Knowledge and appreciation of the challenges in managing vulnerable children and young people.
- The ability to plan, be discreet, confidential, and professional in all aspects of work.
- Excellent organisational and time management skills.
- A flexible and pragmatic attitude with high levels of initiative in identifying and resolving issues.
- Strong and effective communication skills at all levels with exceptional attention to detail and accuracy.
- Flexibility and be willing to work outside of normal hours if required occasionally.

**Hours**
- Part time, 3 days (22.5 hours) a week (with the possibility of becoming a full-time position)

Wage£15,000 - £18,500

**Job Type**: Part-time

**Salary**: £15,000.00-£18,500.00 per year

Expected hours: 22.5 per week
