A vacancy has arisen for a permanent Band 5 Community Mental Health Nurse post within the Birkenhead Community Mental Health Team. We are a dynamic team, working with people where there is an opportunity to significantly impact on an individuals well-being and functioning and long-term clinical outcomes.
Part of the role is to work within the physical health clinics currently being offered so a keen interest in physical health and improving outcomes for people with long term mental health problems would be advantageous.
The service operates from 9am-5pm, Monday to Friday and covers a specific area of the Wirral. The Care Quality Commission (CQC) has rated CWP as ‘Good’ overall, ‘better’ for treatments and ‘Outstanding’ for care
You will be expected to work collaboratively within the physical health clinics currently being offered by CMHT. You will be expected to plan an individual's care needs and assess risk factors and implement interventions that improve outcomes for the individual and their families.
Excellent communication, report writing and assessment skills are essential. We are looking for highly skilled and competent practitioners, who are motivated and committed to delivering quality and evidence based care.
The practitioner will support the development of our physical health monitoring clinics with a vision for quality improvement and have a keen interest in transformation and service development.
You will be an autonomous, independent practitioner who will support the delivery of care for service users who have complex mental health issues within our secondary care services. As this post involves working across a wide geographical area access to transport for work purposes is essential.
CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

All new clinical support staff will be required to have completed or working towards the Care Certificate (CWP In-house 2 day training to be completed within 6 months of start date).
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. You are encouraged to enrol for the DBS Update Service. A small annual fee of £13 per year applies.
New entrants to the NHS will commence on the minimum of the scale stated above.
