Are you looking for an opportunity to provide excellent support to people living with a progressive life limiting illness and those important to them?

In this newly created role, we are looking for an enthusiastic, empathic hardworking individual who will work as part of the Living Well Team, alongside the Living Well Services Manager / Senior Social Worker to enable our Inpatients and Community based patients, plus those important to them, to receive the highest level of home-based social care and support appropriate to their needs.

With a background in health or social care you will be a compassionate individual with experience and understanding of the social care system. You will have excellent communication skills, both verbal and written, which will underpin your conversations with patients, those important to them, internal and external colleagues.

You will be positive and enthusiastic with a genuine interest in the individuals needs and have the empathy, compassion, knowledge, and skills necessary to support them.

Role would be part time - 25 hours over 4/5 days.

FTE Salary is £24000 - £25000 depending on experience.
