We need a skilled, organised, and dynamic nurse who has the flexibility to lead on organisational key initiatives whilst adapting to the changing needs of the Trust and community model of care, as they arise.

Work alongside the locality matron to clinically lead the multi-disciplinary team in the achievement of clinical excellence, patient safety and regulatory assurance. Some of the main duties include:

- Provide clinical & professional leadership to the Community Nursing teams
- Use advanced clinical skills to support and advise on service development and the delivery of evidence-based care in partnership with service users, carers, staff, managers, & other agencies
- Support development of clinical competencies
- Carry out clinical supervision
- Carry out clinical audits
- Lead on investigation and learning from clinical incidents
