The postholder will provide clinical leadership to the Humber and North Yorkshire Diabetes Programme and will be responsible for working alongside the SRO and Programme Lead to: Provide clinical leadership to the development of a three year strategy for diabetes prevention and treatment in Humber and North Yorkshire ensuring that a population health approach is taken Provide clinical leadership for the implementation of the diabetes programme plan, service improvements and any other national guidelines/policies as published; specifically implementation of NICE TAs in respect of continuous glucose monitoring and hybrid closed loop technologies Work with all professional groups, to assess clinical outcomes of developments and agree common standards & protocols Work with the Diabetes SRO and Programme Lead to develop and agree a minimum dataset and analysis of information relating to diabetes activity and outcomes ensuring that a population health approach is taken Demonstrate effective engagement and leadership with all clinicians, nursing and managerial staff including ability to influence, negotiate and support others in times of change and uncertainty Link with individual clinicians and other key clinical leads i.e. medicines management to progress policies and developments agreed at national level and implement within diabetes Support the Programme Lead with the engagement of clinicians in the undertaking of service improvement/transformation projects. Support development and implementation of any relevant enabling plans for estates, digital or workforce linking with the relevant Trusts Understand the changing nature of health care provision, the political climate and financial reality in order to enable the Diabetes Steering Group to position itself in such a way as it continues to meet its objectives In conjunction with the Programme Lead, liaise with place colleagues on investment priorities and service developments for diabetes within Humber and North Yorkshire Support the Programme Lead in the continued focus of the National Diabetes Prevention Programme and the Type 2 Pathway to remission programme (T2DR) and ensure that appropriate connections are made and maintained with CVD prevention leads Link with LMCs on diabetes related matters where clinician to clinician conversations are required Ensure a health inequalities informed approach to system change, supporting the ICS objective of reducing the healthy life expectancy gap across Humber and North Yorkshire.
