**Rethink Mental Illness are hiring a Support Worker to join our inspiring and friendly team at Moultrie Road, Rugby.**

1 x Full-Time (35 hours) position available

**Overview of the service or the team**

Moultrie Road provides a 7 bedded, 24 hour staffed nursing service offering rehabilitation and support to people with severe mental illness. Individuals can be supported to move on into more independent accommodation with the option of move on into our CQC registered Supported Living Service. This service can be provided in one of our 3 bedded properties or in independent accommodation.

All our CQC registered homes aim to help people move on to more independent accommodation by providing support that meets their changing needs. We believe in the involvement of individuals in the management of their own condition. This means offering a stable home life, either for the long or short term, for all those that need our help.

**How will you make a difference?**
- Nursing services are 24/7 services and involve personal care responsibilities - you will undertake basic personal care tasks as outlined in the care plan. These include support with eating or drinking/toileting, washing/bathing/dressing/oral care and the care of skin and hair.
- I provide key worker support to a specified number of service users, including one to one individual support and liaison with other mental health agencies on behalf of people who use our services.
- I always have the individual’s needs at the forefront, working to the recovery model and using communication, motivation and coaching skills to support the service user to achieve their goals and aspirations.
- I develop a rapport with service users based on respect and honesty and work within clear professional boundaries and the Rethink Mental Illness Code of Conduct.
