Onecare has an opportunity for an experienced Care Coordinator who is efficient, organised and a team player to join an established Domiciliary Care and Supported living Business with growth opportunities. This is an exceptionally responsible role, delivering a high standard of accuracy in rota management and monitoring of care support as you will be the first point of contact for staff, service users and commissioning organisations. The role of the Care Coordinator is to support the delivery of quality care services by coordinating care workers including the practical instruction and support of care workers.
- Preparing and sending rotas to service users and care workers. Ensure all calls are covered and making amendments as required.
- Bidding care packages and work with the care manager to pre-empt recruitment needs.
- Accepting, allocating and processing referrals for new work and liaising with social workers and other stakeholders.
- Ensuring all services are compliant with legislations, CQC standards and regulations and keeping computer & paper files up to date.
- Maintaining all office policies, procedures and systems in line with company guidelines.

**Person Specification**:

- Experience in coordinating, rota management and delivering care support services: A person centred approach - 1 year experience preferred.
- Experience of customer care and problem solving.
- Ability to prioritise work under pressure with a can - do attitude.
- Have an eye for detail and is naturally driven to help the team maintain a high standard of service.
- Ability to work independently as well as part of a team.
- Excellent communication and interpersonal skills.
- Knowledge and understanding of principles of good care (CQC Standards).
- Strong IT skills - People Planner, Word, Excel, Outlook.

**Salary**: £21,500.00-£23,500.00 per year
