Humber Teaching NHS Foundation Trust's Award winning YOURhealth service are looking for an enthusiastic, dedicated individual who is passionate about helping / supporting people to become healthier and change their lifestyle as a Social Prescriber. The post advertised will be to working with Nimbus Care Primary Care Network, joining our existing team of dedicated social prescribers.

Social Prescribers give one-to-one and group support, help people to overcome barriers to improve their lifestyle and provide on-going encouragement and motivation.

You will have a sound understanding of the issues affecting isolated and vulnerable people, the ability to motivate people to make changes and have excellent communication skills working within genera practice and supporting the primary care model of delivery to identify and work closes with patients, supporting them to make positive choices that will improve their overall health and wellbeing.
