**CARE COORDINATOR**

Lancing and Sompting Primary Care Network are seeking to recruit a self-motivated and enthusiastic Care Coordinator to join our existing team based across our three practices, incorporating Ball Tree Surgery, New Pond Row Surgery and The Orchard Practice.

As a Care Coordinator you will work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition/s and ensuring their changing needs are addressed.

In addition to practice teams you will also work with social prescribers and other members of the wider multi-disciplinary team. You will need to possess good communication skills and be able to prioritise your work effectively.

Currently the team are supporting the elderly/frail population either by liaising closely with care homes or supporting those in the local community.

Lancing and Sompting Primary Care Network is made up of three-member Practices and serves over 28,000 patients.

**EXPERIENCE AND QUALIFICATIONS**

**Essential**:
To be able to demonstrate commitment to professional and personal development.

Experience of working in Health or Social Care is essential for this role.

**Desirable**:
NVQ Level 3, Advanced or equivalent qualifications or working towards in Health or Social Care.

Training in Motivational Coaching and Interviewing or equivalent experience.

Experience of working directly in either the NHS or Adult Social Care.

**JOB DESCRIPTION**

**Job Title: Primary Care Network Care Coordinator**

**Contract Type**:Permanent

**Hours**:20-37.5 hours per week (2-3 roles up to 2FTE)

**Salary**:Equivalent to Agenda for Change Band 4

**Location**:Various across Lancing & Sompting

**Reports to**:Clinical Director / Practice Manager / PCN Manager

**Purpose of the Role**:
The post holder will be employed by Lancing & Sompting Primary Care Network and deployed within the Primary Care Network (PCN) practice(s) currently known as Ball Tree Surgery, New Pond Row Surgery and The Orchard Practice.

The main focus of the role is coordination of care for a variety of people who may require additional support. This includes, though not exclusively, Nursing and Care Home residents, people with moderate and severe frailty or who are housebound, those with learning disabilities, severe mental health, carers and with long term complex conditions. There is an element of the role that includes working with the social prescribing team supporting their referrals and onward signposting. There is an opportunity to focus on particular areas.

These are new roles and will be co-created with the successful applicant(s) supported by the Practices and PCN Management Teams.

The role involves working very closely with all Practices, Care Homes, Nursing Homes, Housebound, People with frailty and dementia, Carers and Multidisciplinary Teams (MDT) within the Primary Care Network.
