An exciting opportunity has arisen for a Skin cancer and Systemic disease Support Worker for ESNEFT working at Colchester Hospital with scope to occasionally work remotely.

This role involves working within a supportive team environment with the ability to make a real difference to patients with cancer and their families.

You should be confident with effective communication skills, high level of IT skills, supporting senior nursing team, ensuring patients remain on the correct pathway and are seen by the correct clinician within the correct time period. Supporting skin cancer team, attending PTL and liaising with tertiary treatment centres.

Good knowledge of Excel to run reports and audits, good communication skills and knowledge of skin cancer pathways essential.

IT and literacy skills are essential as you will be recording confidential information on a range of computer databases.

The most important element of the role is to assist with providing a smooth service to patients for both Colchester and Ipswich hospital and surrounding areas.

Book/amend/cancel/reconcile appointments using Microsoft Outlook calendar and trust systems, liaising with patients as required.

Maintain the Somerset Remote Monitoring System to include attendance and outcomes of PCFU appointments, ensuring that any patient who does not attend the appointment is followed up.

Monitor investigations via hospital systems to ensure that results when appropriate are communicated to the patient in a timely manner.

Identify any patient who requires follow up investigations to ensure they are managed according to guidelines.

Accept and resolve telephone enquiries from patients regarding their surveillance schedule or generic enquiries, as well as triaging calls for the Clinical Nurse Specialist team and forwarding any clinical queries, to the appropriate Specialist Nurse or clinician.

Have holistic conversations with patients as appropriate and signpost and refer to services, document outcomes on Somerset.

Pre-populate documentation for the Clinical Nurse Specialist, such as holistic needs assessments, care plans and end of treatment summaries.

Regularly review documentation, including leaflets and clinic letters with clinical direction and oversight.

Help and support the PCFU programme by providing a positive and flexible approach to both workload and shift patterns.
