The Integrated Complex assessment team is responsible for the assessing and tracking of patients requiring a discharge to asses (D2A) bed based service within a timely manner relieving the pressures to the acute hospitals.
The Patient Flow Co Coordinator is required to effectively provide support to assist in the delivery of safe and effective patient transfer and tracking within Discharge to Assess Services, working closely with the Discharge to Assess Discharge facilitators offering support and guidance and escalation support.
The Patient Flow Co Coordinator will attend Multi-Disciplinary meetings, prepare reports, spreadsheets as required to enable a robust flow through the D2A beds and escalate any issues or blockages to the Senior Patient Flow.

The post holder should be able to prioritise their own workload to ensure effectiveness and efficiency in delivering objectives whilst working with mínimal supervision. Being proactive and able to demonstrate good organisational skills and excellent communication skills both in verbal and written correspondence.
The role will involve close working with health and social care staff and an understanding of community Home First services alongside knowledge of the Care Act 2014 and the principles of Discharge to Assess (D2A) would be desirable.

Main duties and responsibilities
1. To manage the flow out of the discharge to assess beds.
2. To effectively track the patients journey from referral to discharge in a timely manner using in house IT systems CDS
1. To attend Multi-Disciplinary/ Best Interest meetings to track and update patient journeys which may involve some travelling across D2A footprint to assist with the flow of patients through the service.
2. To work pro-actively offering professional challenge to expedite discharges - reporting any delays or issues to Senior Flow
3. Practical and technical problem solving skills to maintain the flow throughout the service by using own initiative and persuasion at times using relationships and written or verbal communication to reduce length of stay and throughout, helping colleagues achieve the same by sharing knowledge.
4. Able to work under pressure and to deadlines
5. Liaison with families and professionals including external agencies to provide timely updates and discharges with the booking of transport.
6. To monitor and report on LOS with responsibility of meeting proposed targets within the local health economy.
7. Reporting of planned discharges to internal teams to facilitate timely admissions and discharges from acute settings
1. Develop and manage strong relationships with all partners and colleague to support timely updates and flow through discharge to assess beds.
2. To be able to work closely and at pace, assisting other members of the team to meet deadlines, sharing workload and providing cover for absences where necessary, attending and chairing meetings, managing CDS.
3. To provide cover for other colleagues in the event of absences
4. Minute taking as required in family meetings or where needed.
