Pcn Team Leader and Project Support Aylesbury

Key responsibilities Operational and administrative Manage the PCN teams KPIs to ensure goals are being met and support with the deliverables Liaise closely with the 2 practices to help understand the impact of PCN activities on practices in order to assist the Board to meets its objectives and to ensure coherence across the PCN Provide relevant support to the PCN Digital and Transformation Manager and Clinical Director, and Board as the PCN evolves in meeting its contractual obligations to the Network Agreement To assist in measuring and achieving PCN performance against all service specifications and objectives. Supporting the PCN Digital and Transformation Manager in leading on some of these work streams. This includes (but is not limited to) the identified network service specifications as set out in the Direct Enhanced Service agreement: o Social Prescribing o Investment and Impact Fund o Quality Improvement projects o Enhanced Access o Population Health Management o Personalised Care o Anticipatory Care o PCN projects Attend and contribute to all relevant meetings both internally and externally. Ensure best practice is followed in maintaining both professional and clinical standards.
Co-ordinate training and development of PCN staff Collate feedback / analysis data for reporting requirements Support the provision of Enhanced Access, which includes diary management, ensuring the relevant shifts are covered and planning of future clinics
