The Home Treatment Team based at St Ann’s hospital is looking for an experienced Community Mental Health Practitioner on a full time basis 37.5 hours per week, to join our fun, friendly and fabulous team, who have patient care at the heart of all that we do

The Home Treatment Team East (HTTE) offers an alternative to hospital admission for service users aged over 18 with mental illness through intensive home treatment. We pride ourselves in working flexibly with patients and supporting their carers when in a mental health crisis.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

To be personally responsible and professionally accountable

for working with people in the community with severe mental illness who have multiple and complex needs.

To provide high quality care and treatment in conjunction with other professionals through the Trust’s agreed multi
- disciplinary assessment and treatment/ care programmes.

To participate as an active member of the Home Treatment Team and take a lead role in providing high quality and innovative services to people with severe mental illness.
