**We area residential home for people with learning disabilities, autism and behaviours that may challenge.We support individuals from the age of 17 as part of a transitional model of support. In each case, Rhyme house works closelywith all parties to ensure an agreed approach is put in place to help an individual make the transition to adulthood a positive one.**

**Your new job**: Support workers treat everyone equally and with the dignity and respect they deserve: you’ll play a vital part in providing support that improves the ability to manage daily tasks and enable people to experience independentliving and decision making.

Your role is to:

- Support people in achieving their goals and ambitions
- Planning and engaging in activities which include sports, day centre activities, trips to the cinema, bowling, visiting the beach, arts and crafts, music crafts and many more.
- Make a positive contribution to the lives of others
- Prompting and/or supporting personal care
