The Linden Lodge Reablement team provides specialist rehabilitation to adults in the community following Discharge from Linden Lodge On the city campus and the regional neurosciences unit at QMC.
We are looking for an enthusiastic and committed individual with exceptional communication skills to join our team as a Therapy Assistant Practitioner.
The Therapy Assistant Practitioner will work closely with the MDT to provide rehabilitation in the home and other community settings. The post holder will deliver pre-defined intervention without direct supervision from a clinician in the patients’ own home or community setting.
You will need excellent time management skills and possess experience of working in a rehabilitation environment. Experience of neurorehabilitation is desirable.
You will need to be flexible and assist colleagues to cover service needs. This will include travel across the whole of Nottinghamshire to provide patient intervention. You will also be required to support the team with administrative tasks. You will also need to be computer literate, with a willingness to learn how to use of a range of hospital ICT systems.
The post holder will receive regular supervision, access to training and support to achieve personal development objectives.

You must have a passion for delivering patient centered care and a desire to provide support to a small but diverse MDT including face to face and remote patient care..
There is an expectation that you will be actively involved in project work & engage in ongoing service developments, working closely with community colleagues around the discharge and ongoing rehabilitation of patients.
You will work alongside the MDT in the assessment, intervention and delivery of care patients, as well as delegated admin duties..

Therapies and Rehabilitation Services is one of the pathways that together make up the Clinical Support Division at NUH. We are a diverse pathway of AHPs and support staff, employing over 600 staff who work together to deliver the high quality care we aspire to.
Our HCPC registered AHP professionals comprise of physiotherapists, dietitians, occupational therapists, orthotists, prosthetists and speech and language therapists, and our other staff encompass professionals such as nurses, psychologists, administrative and clerical and support workers.
We have a Chief AHP to lead us on our strategic direction, and many of the pathway leadership team have pathway AHP backgrounds to support our staff. There are also Practice Development Therapists to support career development for all.

Undertake evidence based assessment of patients, having a flexible approach to the delivery of care.
Develop evidenced based holistic treatment plans for individuals, enabling individuals, carers and families to contribute to the plan.
Establish effective working relationships and communicate with the wider MDT, patients, families and carers.
Proactively advocate for patients.
Proactively work with patients promoting self-management empowering them to make choices about their future management.
Liaise with clinical colleagues from all specialties regarding patient needs
