An exciting opportunity has been created for a Mental Health Practitioner to work within the Miles Platting and Newton Health Primary Care Network across Manchester. The post holder will work alongside the Senior Mental Health Practitioner to provide a combined consultation, advice, triage and liaison function, supported by the local community mental health provider and will work with patients to: (1) support shared decision-making about self-management; (2) facilitate onward access to treatment services; (3) provide brief interventions, where qualified to do so and where appropriate.
