The Service for Ageing and Mental Health in Camden are delighted to offer the opportunity to join our Community Mental Health Team to provide support to those with severe and enduring mental illness across the borough.
As a service we pride ourselves in working with specialist knowledge and experience to support individuals and their community network, including carers. We work as a multi-disciplinary team (MDT) including; Psychiatric nurses, social workers, psychologists and assistant psychologists, OT and psychiatrists. You will be someone who has an ability to build rapport with members of the team, wider communities and users of our services. We are looking for a team player as well as somebody who is comfortable with working autonomously, under the direction of qualified professionals within the team. You will be knowledgeable of the care and support needs that this client group may have, including the impact of the ageing process and comorbidities effecting both physical and mental wellbeing.
The post holder is responsible for participating and assisting in the delivery of assessment and on-going support to service users, family carers and other professionals within the Camden Community Mental Health Team, within the Services for Ageing Mental Health Service. They will be responsible for a caseload of patients living with range of functional mental illness in the community and will be undertake regular reviews, monitoring and interventions of practical support alongside other members of the team.
They will work as part of the multidisciplinary team to provide excellent, compassionate and sensitive care to people who have significant mental illness and their carers.
Working under supervision, the post holder will provide direct support to a defined group of service users as allocated and delegated by the Team Manager or senior member of the team.
The focus of the role will be that of supporting older people with mental illness enabling them and their family carers to live well and to support with reviewing their social care needs under the Care Act (2014).

We also develop good working relationships with voluntary and groups in our local areas. The role will also include some administrative duties to ensure the smooth and efficient running of the clinical environment and practical assistance to individual practitioners and doctors in the assessment and review of service users.
