We are excited to be able to recruit additionalRegistered Nurses or Therapists to join our existing team of Specialist Frailty Practitioners (nurses and therapists) in delivery of the King’s Frailty service over the coming months.

The Specialist Frailty Practitioner will deliver person centred care, frailty screening and comprehensive assessment for older people and people with frailty within the Emergency Department and across the Frailty Pathway at King’s College Hospital. These fixed term posts will include weekend working as we expand to a seven-day service therefore the postholder must be available to work within a flexible rota.

- The Specialist Frailty Practitioner is a member of King’s Frailty Team, delivering multidisciplinary assessment and clinical management support for frail patients attending the Emergency Department and acute services
- The Practitioner will carry out screening and specialist assessments of older people in the Emergency Department and acute service using a Comprehensive Geriatric Assessment model
- Plan and deliver interventions with the aim to facilitate safe and timely discharges and prevent avoidable admissions
- Promote excellent integrated, multidisciplinary and multi-agency working.
- To ensure that patients and their families are fully involved, informed and supported at all stages of the patient journey
- To establish clear lines of communication with primary healthcare teams and outside agencies in order to promote optimum service provision
- This post supports Winter Pressures arrangements for seven day service and therefore is fixed term and will require the postholder to undertake regular weekend working
