The posts is a Full Time, 37.5 hours within West Early Intervention in Psychosis Team, covering Thurrock, Basildon, Brentwood and Wickford in Essex. Work flexibly across the West ESTEP Service and participate in 7-day rota.

You will be based at Grays Hall, in Grays, Essex, however, would be expected to travel and also work from other Trust sites, such as Pride House in Laindon, Basildon Hospital, Brentwood Community Resource Centre

As part of this team, you will be expected to care co-ordinate and work with clients with a first episode psychosis aged between 14-65 years and their families.

We work very closely as a multi-disciplinary team and expect all team members to support our commitment to a programme of education and awareness to carers and in the local community

Using the principles of creative engagement, you will be involved with the MDT in providing psychosocial interventions, Behavioral Family Therapy, Relapse prevention, Assessments, and care co-ordination to individuals with complex mental health needs, their Carers, families and friends in order to promote and maintain their recovery and social functioning. This will help them build their own support network and engage fully with life. Our emphasis is on making every visit count.

The band 6 role in the team is an important asset to the multi-disciplinary team and a role that will help to shape the delivery of care to clients with first episode of psychosis and complexity of need.

**The main role responsibilities are**:

- Care co-ordination.
- Participate in theTeam Duty Roster and initial assessment of clients to determine appropriateness for ESTEP and to liaise with referring agencies to identify alternative agencies as appropriate to meet the client’s needs.
- Prioritize and manage workload to ensure the effective management of a defined group of clients with complex needs.
- Work with MDT in seeking share risk management and to improve the quality of services provided to our clients/patients.
- To provide clinical supervision and professional leadership to junior staff and promote evidence-based practice.

Development opportunities exist for band 6 nurses in non-medical prescribing, Approved Mental Health Practitioner Training, CAARMS training, and further courses such as, Cognitive Behavioral Therapy for Psychosis (Full Training or Top Up for staff with previous CBT/DBT training) - Family Intervention for Psychosis and Bipolar Disorder.

- To undertake care co-ordination of a small caseload of clients.
- To participate in the initial assessment of clients to determine appropriateness for ESTEP and to liaise with referring agencies to identify alternative agencies as appropriate to meet the client’s needs.
- Carry responsibility for the assessment of care needs, the development, implementation and evaluation of recovery-based care plans.
- Participate in collection of data including looking at entry and exit pathways to care, and duration of untreated psychosis as well as Dialogue & QPR to ensure that we provide a successful service in terms of high-quality outcomes.
- To maintain effective individualized care through assessment of service user and carers problems. Plan, implement and evaluate interventions and clear, accurate and concise documentation of individualised care.
- Prioritize and manage workload to ensure the effective management of a defined group of clients with complex needs.
- Following analysis of client assessments, plan creatively to meet their needs including the provision of evidence-based interventions.
- Responsible for working with colleagues in seeking to improve the quality of services provided to our clients/patients.
- To provide clinical supervision and professional leadership to junior staff and promote evidence-based practice.
