We are looking for B6 registered staff who want to make a difference in helping people make the most of their lives and avoid admission. The Dementia Intensive Support Team in North East Essex provides a range of supportive interventions in the persons home to prevent and manage crisis. DIST has been in operation for around a year and we are looking for someone who can contribute to the good work, innovate new ideas, help drive service improvement and work collaboratively with a number of individuals and organisations to reduce the number of admissions for this group.
Applicants will be expected to have professional registration and relevant experience at B5 or above.
KEY RESPONSIBILITIES CLINICAL
The post holder carries responsibility for the assessment of care needs and the development, implementation and evaluation of programmes of care for patients with dementia who are within their own home a care home, nursing home or present within Colchester Hospital or within in EPUT.
The Dementia Intensive Support Team provides short-term intensive treatment in the person’s place of residence. Working in close co-operation with hospital, community services, primary care staff, social care professionals and third sector organisations e.g Alzheimer’s Society, Age Concern. They will be responsible for delivering treatment and helping to maintain standards of care within the Team. They will be expected to work collaboratively with individuals and their carer’s/family delivering person centred care. Actively participate in the training and education of staff, families and carers as required. In addition, they will also be responsible for ensuring that an integrated service is provided and ensure that good inter-disciplinary working is maintained.
