**Company Description**:
Mr R.K has been a client of Home Instead - Camden since the January of 2021. Currently, Mr R.K is deteriorating, family, and Mr R.K have agreed to resume with a Live-In.

Mr R.K lives in Hampstead, is very gentle, calm and enjoys conversing in conversation. Important to note that due to living with Dementia, Mr R.K may show signs of confusion, asking the same questions, in return Mr R.K will require reassurance.

Mr R.K is a Cambridge graduate, studied Law. Throughout his life, he has travelled the world for work purposes, owned own business, lived abroad in places like India, Pakistan, Greece and France.

Mr R.K is very supportive by his children, as of now, Mr R.K is living with wife Anna. Anna is very capable to look after herself, runs own business, spends a lot of time in her office, and attending various meetings in London.

Anna and Mr R.K have agreed to resume with a Live-In-CARE package, see below main factors:

- CAREGiver to have the skills and the ability to cook foods from scratch. Anna enjoys cooking, ideally Anna and CAREGiver to discuss foods, planning. Encourage water/fluids to Mr R.K.
- CAREGiver to carry out tasks involving light house duties, such as; hoovering (when required), laundry (when required), dusting (when required). Mr R.K and Anna have a domestic worker that attends to household chores, allocated twice a week. CAREGiver to have own initiative when carrying out necessary chores. For example, when laundry is required, CAREGiver to act up on it, and not wait for the domestic worker to complete the chore. Working as and when required.
- CAREGiver to administer medication to Mr R.K, checking blood pressure, checking oxygen levels, reporting to the office as of when needed. Writing daily logs, completing MAR charts. CAREGiver will be shown accordingly how each task should be achieved.
- Mr R.K will require assistance with toileting (throughout the day), emptying urine bottle - when needed. Mr R.K is very private, will do his best to do most tasks on his own. CAREGiver to be on standby.
- Depending on how Mr R.K feels, though this can vary day to day. Mr R.K and CAREGiver to go out walking, Mr R.K uses a three-wheeler for outdoor events, on Thursday(s) Mr R.K goes to local church, this is an opportunity to meet all the people from the neighbourhood.

**Job Description**:
Mr R.K is very capable to memorise most things that have occurred, however, Mr R.K may muddle up things due to memory loss, therefore patience, offering reassurance is very crucial.

Mr R.K will require support/assistance with the following:

- personal hygiene,
- personal care,
- drying,
- medication,
- assistance with mobility,
- offering companionship,
- assistance with foods - cooking from scratch / encouraging water/fluids.

**Qualifications**:
Mr R.K has the following _equipment_:

- **Wheelchair** - this is used when Mr R.K is feeling under the weather and may require assistance with manoeuvring.
- ** Z-Frame**

Soon, Mr R.K will be seen by the OT (_occupational therapist_), new suggestions' may be added. CAREGiver willing to be adaptive, having the ability to learn and improve as and when needed.
