Pitlochry Community Hospital
A permanent vacancy has arisen for a Community Care Assistant in the Hospital Discharge Team, working 23.2 hours per week / 23 hours 12 minutes per week, and we are negotiable regarding your working days.
The Hospital Discharge Team work to facilitate, safe, timely and person-centred discharges from hospital for individuals who require support once discharged.
This is an excellent opportunity to join a well-established, highly skilled and motivated team, with excellent training, supervision and development opportunities. Benefits include flexible working, along with supervisory/ mentoring support.
The client group supported by the team, encounter a diverse range of care requirements. The team are embedded within an approach of multi-disciplinary working, with the aim of considering a holistic approach, ensuring clients and families are at the centre of discharge planning.
The successful applicant will assist in ensuring discharges from hospital in line with current principles of discharge without delay. Focussing on the right care within the right place. Therefore, implementing best practice for facilitating smooth transitions from hospital to community.
Tasks include undertaking Outcome Focussed Assessments, negotiating flexible packages of care, delivered via Self Directed Support (SDS). In addition, maintaining an awareness of the rights of cares to be informed and involved in decision making, including a focus on carers own individual requirements.
**Skills & Experience Required**:
Effective time management is essential for a successful autonomous worker to manage complex caseloads with the ability to be flexible, responding to competing demands being an essential element of the role.
An in-depth understanding of Self-Directed Support and how this is delivered would be advantageous.
The ability to integrate and cultivate successful links within the team and partnership colleagues is essential.
Knowledge of services demands and practice challenges, including capacity and flow within the hospital shall be a key factor within knowledge development.
You must have experience and skills of client needs assessment.
You will be required to demonstrate a sound knowledge and understanding of social work principles and values.
You must have or be willing to work towards SVQ3 in Health & Social Care.
You must have a valid driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.
