The Community Urgent Rapid Response Team provides holistic person-centered care with the aim of preventing unnecessary hospital admissions and facilitating timely hospital discharges in a variety of ways. The team enables individuals to increase independence, improve health outcomes and reduce unnecessary admissions to hospital, long term care and minimizing social care packages, by designing programs for recovery and self-care management for the adult population of Bassetlaw.

This is achieved through timely assessment, interventions, rehabilitation, management of long-term and life limiting conditions, and partnership working across the health and social care communities.

An exciting opportunity has arisen for an Assistant Practitioner to work within the Urgent Care Rapid Response Service provided by Local partnerships in Bassetlaw.

You will be providing support to the team Clinical Assessors to provide clinical triage and navigation, urgent response assessments and facilitation of discharge from the acute provider, focusing on the hospital avoidance agenda, discharge to assess and 2-hour/2-day guidelines and promoting flow through the system.

Working alongside our community health colleagues in other teams and acute trusts to support discharge and prevent hospital admission. You will work in a large and supportive multi-disciplinary clinical team with a strong identity and dedicated focus of excellent patient care and innovation.

You will liaise and coordinate services closely with other health and social care providers to enable recovery from illness, promote health and wellbeing and foster self-care.

The Community Urgent Rapid Response Team prides itself on setting the highest of standards of patient care.
