We’re looking for **Support Coordinator** to join our **Housing Service**, supporting individuals in the community with:
- Mental health challenges
- Trauma and addiction recovery
- Social isolation and tenancy sustainment
The Coordinator plays a key role in leading individual shifts while supervising and providing practice and reflective support and personal development plans to less experienced staff, supporting them to develop and adapt their skills and knowledge to meet the often complex and changing needs of the people they are supporting.
Undertaking a wide variety of tasks in a fast-paced health & social care setting requires the post holder to have the ability to flexibly respond to individual needs and aspirations, and to assist individuals to have the maximum opportunities to direct their support or care to live as independently as possible.
**As a Coordinator you will also**:
- Involve the Supported individual in devising, reviewing, and updating their service engagement agreement and recording events and observations.
- Support and contribute to the resourcing of interventions for people we support in crisis situations, and/or manage physical risk or behaviour likely to cause incidents, in accordance with the individuals personal developmental support plan, risk management plan or service protocol.
- Provide support in a way which equips individuals with the confidence and skills to move on to more independent and sustainable living.
- Attend and contribute to individuals reviews often as a key worker or as required.
**Essential experience, knowledge and skills include**:
- Sound knowledge and experience of the challenges and needs of vulnerable individuals
- An understanding of the impact of alcohol or drug misuse on individuals, families, and communities
- Experience of working in partnership with statutory agencies and of facilitating groups/training
- A creative approach to problem solving and a good negotiator
- Active listening skills and interpersonal skills
- Experience of coaching, supervising, or managing staff within a health & social care community setting
- Excellent written and oral communication and proficient computer skills
- Excellent time management skills and the ability to prioritise and organise
This post holder should have an appropriate health and social care qualification at level 3 or equivalent professional qualification.
