Community Complex Discharge Tracker

Scheduling and allocating care calls on the Complex Discharge Tracker System.
- Working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales.
- Ensure that the Complex Discharge Tracker system is validated and updated within each locality team.
- Review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.
- Dependent on capacity and demand of the Home First Teams ensure that any unmet referrals are immediately escalated for authorisation by the Registered Manager for the individualised plan of care to be sourced from external partners.
- Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
- Keep manual backups of patient exports at regular intervals in case of system downtime expected or unexpected.
- Attend all CDT training and briefing sessions as the system evolves.
- Ensure that any and all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.
The Home First Team is an integrated clinical and care and support service that provides short term reablement intervention to enable people to maximise their independence and meet their health and social care needs within Midlands NHS Partnership Foundation Trust (MPFT).
The post holder will be responsible for the scheduling and allocating care calls on the Complex Discharge Tracker System. They will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales. They will ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.
The successful applicant will receive direct line management and will be delegated team work priorities from the Home First Shift Leads. The post holder will be enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills.
The post holder will be expected to commute to the office bases across North Staffordshire, if required.
The shift patterns are on a rotational basis over 7 days, early shift 7am - 3pm and late shift 12pm - 8pm working 2 weekends a month, therefore flexibility is required for this role.
- To work as flexibly within a team and create good relationships with colleagues to assist in the smooth running of the service
- To participate in the appraisal and personal development process to identify their own needs and how they can be met.
- To support staff with raising concerns and reporting incidents as per Trust policies.
- Comply with and ensure staff are aware of organisational policies, procedures and legislative frameworks.
- To work within infection control guidelines to ensure that support given does not constitute a risk of infection either to the worker, or the individual whilst working in the individual’s home
- To be aware and ensure that staff comply with all Trust policy requirements in relation to Information Governance, including the Freedom of Information Act 2000, the Data Protection Act and the Trust’s Electronic Record Management Standards & Guidance.
- To be flexible to undertake any other duties that may be allocated from time to time that is appropriate to the position and support the service needs.
