Elevate Housing and Support LTD is a supported accommodation provider for young people leaving care. Our home is staffed 24 hours a day, we provide support to young people aged between 16-21 years old who may be vulnerable and have varied support needs. To support those identified needs we deliver a range of holistic based bespoke support packages that are tailored to each individual.
Areas of support provided but not limited to; health, education, training, employment, budgeting, maintaining and managing a home environment, and relationships. With our support and guidance, our mission is to prepare each young person with the necessary skills for their transition to independent living.
**Job description**
The staff team adopt a person centred, solution focused approach. Staff are innovative in their approach and regularly adapt their practice to meet the needs of the individual. Our staff team are strong advocates for the young people that we support.
The key tasks for the support worker are:
- Contribute towards the general running and organisation of the home and its related activities
- Accurate reporting and recording of activities in the home daily
- Recognise and report safeguarding concerns within timescales
- Share responsibility for the safety and well-being of all young people in the home
- Liaise and build relationships with external agencies such as social services, health care, youth offending teams and education providers
- Building positive relationship with young people
- Observing young people for behavioural changes and respond appropriately
- Promote engagement and participation
- Attend regular staff and supervision meetings outside normal working hours
- Accompany young people to their appointments
- Carry out health and safety inspections and duties
- Work with health and social care professionals and external agencies.
- Provide 1:1 support and group support sessions
- Attend and contribute to multi agency meetings
- To input into young people’s Pathway plan, statutory reviews and Education, Health and Care Plans (EHCP)
- Develop support plans to meet identified needs
- Providing intensive, high level practical support in response to crisis/relapse situations
- Take part in mandatory youth work training, and necessary training to further enhance skills and development
**Requirements/ Qualifications**
- Must hold a (NVQ) Level 3 Children and Young People's Workforce
- Have the ability to communicate effectively with young people and adults in a positive way
- Can work confidently in a team and independently
- Due to the nature of this post, the preferred age of employees is 21 and above
- Employment unable to commence without satisfactory DBS check undertake
- Good communication skills, both written and verbal.

- A-Level or equivalent (preferred)
