ADSS - A local Charity based in Northfleet, Kent, is an organization that has been pivotal in helping support people in our community to live well with Dementia for over 30 years.
We have a fantastic opportunity to be part of a well established team, who share a passion for inspiring the people who work with us to provide high quality, person centred support to people affected by dementia. As an experienced Supervisor/Team Leader, you will provide direct line management to a team of Support Workers, as well as a managing a caseload of individuals whose needs may change quickly or fluctuate, with the expectation that individuals' Care Plans reflect an individuals' changing needs and how best to support them.
You will be responsible for assessing risks (in people's homes, before the commencement of outings/activities, when medication administration support has been identified etc) and communicating clearly, how best any risks identified can be minimised.
This role is a full time role (37.5 hours per week) and as part of your working hours, you will also provide On Call cover once a week. You will also be required to work one weekend in 4.

**Who we're looking for**:
Experience of supervising a team is essential.
Experienced with Microsoft Office Suite.
Significant experience of creating and maintaining Care Plans, conducting risk assessments and auditing of compliance data.
**Main Responsibilities**:
You will be motivated by being part of an exceptional team that works tirelessly to improve the lives of people affected by dementia. You will be an excellent team player and people affected by dementia will be at the heart of every action you take and decision you make.
1. Ensure the professional direct delivery of core services, as directed by the Dementia Care manager, including rotas, striving to maximise efficiency and minimising costs, whilst maintaining quality assurance, compliance and continuous improvement against CQC, legislative and organisational standards.
2. Proactively seek feedback and evidence of good practice, to help with evaluation and continuous improvement that identifies how a desired outcome of an individual is to be achieved, whilst promoting choice and independence in line with our organisational values.
3. Monitor and evaluate services, reporting agreed KPI’s on a monthly basis to the Dementia Care Manager.
4. Ensure, with the Seniors in your team, that assessments for new services and reviews of existing services are completed in a professional, compassionate and timely manner, ensuring that people’s abilities, needs, wishes, and desired outcomes are clearly outlined in the individual’s Care Plan.
5. Facilitate bi-team meetings for Care Workers to update on service performance, policy changes, charity news and developments, alongside the Senior Care Workers.
6. Support the recruitment and induction process when required and actively engage with and encourage the professional development of Care Workers.
7. Support the Dementia Care Manager to promote the wellbeing of staff by communicating concerns, safeguarding reports, compliments and complaints, whilst ensuring the company’s policies and procedures are followed.
8. Manage the coordination of supervisions, return to work interviews, absence management reviews and appraisals, in line with internal policies.
9. Actively promote independence, choice, confidentiality and dignity and lead by example and develop good working relationships with external professionals and other community services to ensure the best outcomes are achieved for the people we support.
10. Provide out of hours On Call support alongside the Senior Care Team on a rota basis.
11. Work in partnership with the Dementia Support Team and other professionals to ensure individuals get the support they need.
