Working within Keswick & Solway’s or Eden’s GP practices and working closely with the wider ICC team, the Living Well Coach (LWC) will work within the Wellbeing Service and target and provide assessment for individuals with LTC, Frailty and psycho-social needs engaging them to set personalised goals and find and access services and resources best suited to meet their needs and circumstances.
Depending on the individual the assessment might include basic health assessment, low level clinical screening, wellbeing assessment, review of functional abilities, assessment of social / housing / employment circumstances and gaining understanding of social and support networks.
LWCs will develop skills in motivational interviewing and appropriate psychological care skills including CBT, acceptance & commitment therapy, mindfulness. Training and on-going clinical supervision will be provided by the Physical Health Psychology Department within North Cumbria Integrated Care NHS Foundation Trust (NCIC).
Post is initially offered as fixed term, but may become permanent.
To meet the requirements of the individuals identified as within the Frailty category the LWC (in appropriate cases) will undertake home visits and undertake a holistic assessment including use of the agreed Falls Risk Assessment Tool to identify those patients appropriate for referral on to specific community services. Whilst these patients may already have a history of falls, they may also be those who have not fallen but who would benefit from preventative rehab input. The LWC will work closely with the Community Rehab team to increase referrals into higher level specialist falls assessment and rehab services. The overarching aim of the role of supporting individuals classed within the Frailty Category is to reduce the incidence of falls across Keswick & Solway or Eden.
Although the base will be within one/ two GP practices in the Keswick & Solway or Eden Integrated Care Community (ICC) the post-holder will be expected to work in a range of environments including visiting people in their own homes, in other GP practices or community health/ care settings. Individual LWCs will be allocated to one or two practice(s) allowing continuity for patients and direct engagement with the GP practice team.
