About the role
An exciting opportunity has arisen for a post within our Prevention Hub for a Prevention and Early Intervention Liaison Coordinator.

This role involves triaging referrals through to the prevention services, ensuring that information is correct and analysing data and collating and disseminating performance data internally and externally to acute trusts.

What will you be doing?
You will be working within the prevention hub to triage referrals coming into the teams, clarifying information with professionals, inputting into internal systems and arranging start dates for care support. You will also be making referrals to external teams.

You will be working within the local hospitals of Addenbrookes/ Hinchingbrooke, supporting social workers in completing reablement referrals.

About you
You will need to be fast paced, organised, proactive in your approach to demand, and be able to work autonomously using your own initiative. You will also be required to undertake some travel between other location offices for meeting, and so will have the means to travel. You will be required to work weekends on a rota basis.
