We are looking for part time Key workers to work in our care home based in Blackburn, with starts ASAP.

1. **Promoting and Supporting Rehabilitation**

1.1. To work with residents in a person-centred way, following the Recovery Star Model in order to support them to follow a rehabilitation programme in accordance with individuals’ desired outcomes.

1.2. To be involved in the assessment, planning, monitoring, implementation and evaluation of allocated residents’ recovery plans and risk assessments.

1.3. To support residents to consider previous risk factors including alcohol and drug use and implement relapse prevention strategies.

1.4. To support residents to develop daily living skills such as shopping, budgeting, household management and to meet their personal care needs as necessary, taking into account ability and promoting independence.

1.5. To support residents to manage their medication regime and support their progression to self-medication.

1.6. To support residents to understand and meet their healthcare and nutritional needs whilst respecting their individual choices.

1.7. To encourage, promote and support residents’ engagement in meaningful activities in accordance with their interests.

1.8. To encourage, promote and support residents’ engagement in their community including through voluntary work and education.

**2. **Communication**

2.1. To liaise with team members regarding residents’ needs and goals on an ongoing basis including attendance at and contribution to team meetings.

2.2. To protect the confidential nature of the work.

**3. **Managing Information and General Administration**

3.1. To complete residents’ notes and monitoring records to a professional standard.

3.2. To assist in the compiling of reports for the purpose of CPA review meetings and other professional meetings.

3.3. To ensure that all other documentation including medication records is completed accurately.

**4. **Meeting Legal and Safety Requirements**

4.1. To follow Almond Villas’ policies and procedures.

4.2. To ensure that working practices meet legislative requirements as defined in The Health and Safety at Work Act 1974 and the Care Act 2014.

4.3. To follow Blackburn with Darwen’s Safeguarding Adults and Safeguarding Children’s procedures regarding the reporting and recording of abuse or suspected abuse.

**5. **Training and Development**

5.1. To attend regular supervision sessions.

5.2. To attend all relevant training courses required for the role including mandatory courses**.**

5.3. To be pro-active in developing own knowledge base in relation to residents needs and the requirements of the role

**6. Job Specifics**
- 25hrs per week
- 1 Sleep-in shift every 2 weeks
- Working alternate weekends
Applicants must be 21yrs + and must be fully vaccinated

**Job Types**: Part-time, Permanent
Part-time hours: 25 per week

**Salary**: £8.95-£9.00 per hour
