MMP Consultancy are currently recruiting for a Floating Support Worker on a temporary basis, based in London.
The client provides flexible housing related support across all tenures, it aims to enable service users to continue to live independently within their own home, regardless of tenure type. The Floating Support Worker will support a caseload of clients withina recovery framework. The focus is on addressing housing related issues, improving quality of life and promoting independence.
The Floating Support Worker will carry out a comprehensive assessment of needs and risks. Ongoing high quality support plans and assessments will then be drafted and reviewed on a regular basis. All work undertaken will be in line with planned support, makingsure that clients are actively involved and that the support offered is full and holistic.
Key Responsibilities:

- Work in partnership with the client and any significant others involved in their support, to ensure assessed needs are translated into action plans that are SMART and give scope for progression
- Produce comprehensive and high quality risk assessment and risk assessment plans
- Minimise risks to clients by identifying, reporting and following up any safeguarding concerns and incidents
- Support clients to access activities which will assist them in working towards gaining greater independence within the wider community
- Participate fully in rotas covering duty sessions and any related service activities
Key Knowledge & Experience:

- Experience of supporting clients to carry out tasks such as budgeting, calculating benefit entitlements and understanding rent arrears and housing options
- Experience of working in a client centred support service working with vulnerable people presenting significant levels of need and risk, with an ability to devise and deliver on action plans though high quality risk management and needs assessments
- A demonstrable level of experience and understanding of the range of approaches appropriate to working with issues such as substance use, mental and physical ill health, dual diagnosis, challenging behaviour, risk of homelessness, offending, antisocial behaviour,social isolation, domestic violence, hang-relate issues, young parents, leaving care and learning disabilities
- The ability to effectively liaise with a range of service providers and agencies in order to establish or improve services for clients
