This is an exciting opportunity to join our existing Transitions Team providing wellbeing support and intensive client work across the Wakefield district. The Wellbeing Outreach Service provides an enhanced community-based outreach support service to young people aged 12-19 who are identified as SEND and facing mental health difficulties impacting their ability to engage with educational provision. This group may include young people with a mental health diagnosis, those on waiting lists with lower level support needs, those living is difficult family situations, which all impacts their ability to engage or attend educational provision or positive activity.
- Young people referred to this project may include young people identified as SEND (including
- autism) who:
- Would benefit from community outreach support as they are unable to access mainstream provision
- Are unable to engage with education and positive activity due to impact of social isolation including (but not limited to) anxiety and low mood and therefore need help to attend and engage
- Require ongoing support to build positive relationships with professionals, build their own confidence and resilience to allow them to better access mainstream support.

This role offers a unique opportunity to provide individually tailored support for young people for up to 12 months, enabling them flexibility and opportunity for 1:1 support, for example they may need practical support in leaving their home, travelling around their local area using public transport, accessing GP services, local community venues, engaging in the community and other local support groups and building a peer network. This holistic approach means the role can be extremely varied and lead to working with the individual across a number of settings and with a number of different agencies and organisations to best support that young person. Offering such a variety of support means this role will have more opportunities for learning, training and personal development to ensure the successful applicant feels confident and competent to support the young person across a variety of areas.
- Wherever possible for sustainability and support, we are keen to make community links and partnerships, therefore the successful applicant will have the exciting role of continuing to build on new and existing relationships across Wakefield and support our Senior Transitions Worker to map and build on our knowledge of community provision and support as well as developing our signposting, community engagement and onward referral links.
