**The Client**

We provide wide-ranging NHS care for people in the community, in a range of settings including people's own homes; nursing homes; health clinics; community hospitals; minor injury units and in mobile units.

We are one of the largest NHS community health providers in England, serving a population of about 1.4 million across Kent and 600,000 in East Sussex and London. We employ more than 5,000 staff, including doctors, community nurses, physiotherapists, dietitiansand many other healthcare professionals.

**The Role**

Our Urgent Therapy Stream provide reactive care to enable people to stay in their own home during period of crisis. The team work to respond to referrals same or next day, completing holistic assessments of patients in their home environments and deliveringmoving and handling solutions to make the individual safe, and optimise their functional independence. The therapists work closely with our urgent care multi-disciplinary services including, Rapid Response, Home treatment Service and Hospital at Home. Workingalongside Nurses, Doctors, Advanced Clinical Practitioners and Support Workers. Therapy peer support is provided through our established Community Rehabilitation Team made up of Occupational Therapists, Physiotherapists and Therapy Assistants and Apprentices.Due to the nature of this work the service is required to cover a seven-day service over extended hours (8-8) in order to support the target 2 hour response work developing currently in our urgent care services. Therefore, flexible working patterns can besupported. You must be an HCPC registered Occupational Therapist and will be required to travel around the Trust in a timely manner so access to a car and full driving licence is essential.

**Skills & Attributes**

Sound OT skills including Moving and Handling assessments experience, communications skills, teamwork. Rio knowledge beneficial not essential.
