Vascular Access Coordinator

As a vascular access coordinator your role will involve working closely with and supporting the vascular access nurses and the vascular access team as a whole. The main focus of the role will be managing and coordinating the patient clinical pathway to ensure patients have a smooth, timely and positive experience. Your personal and professional experiences and skills will be valued and you will be able to show case your communication, interpersonal, managerial and computing skills. You will have ample opportunities to learn, share your knowledge/skills and grow with the team. Having a clean driving license will come in handy.
Responsible for the management of the surgical and nurse led assessment clinics to ensure patients have a timely and positive experience and meet expected standards of the referral to treatment (RTT) process. Work closely and support the vascular access nurses, the vascular access team and the wider multidisciplinary team across the renal unit. Assist the dialysis clinical staff ensuring patients clinic appointments do not clash with their treatment schedule. Collect data related to clinics non-attendance, surgical and radiological procedures and produce reports to be discussed in regular team meetings. Exhibit exemplary behaviour and attitude, display flexibility, work in a collaborative manner and demonstrate excellent customer care skills. Ensure clear and effective communication with patients, relatives and colleagues at all times. Observe Renal Unit and Trust standards, protocols and guidelines in all aspects of service delivery

**The post holder will**:
- Work closely and support the vascular access nurses to ensure the patient vascular access pathway is effectively and efficiently delivered.
- Have good knowledge and understanding of the referral to treatment pathway (RTT) and take sole responsibility for booking patients into the surgical assessment and nurse led maturation clinics.
- Keep accurate records of all Electronic Patient Referrals (EPR), ensure patients are booked into clinics according to referral urgency and monitor non-attendance rates.
- Work closely with clinicians and assist in data collection of all surgical and interventional radiological procedures.
- Be prepared to stand in for admissions’ coordinator when required.
- Be the first point of contact for patients, relatives and clinicians enquiring about clinics dates and times.
- Ensure all clinical outcomes are completed and correctly documented on PIMS. Send out appointment letters to patients in a timely manner.
- Liaise with clinicians across the renal unit and relay accurate information related to the vascular access pathway.
- Assist the access nurses in the organization and prioritisation of the renal vascular surgery lists, the interventional radiological procedures, communicate with patients, dialysis nursing staff and ensure coordination of patients’ clinic attendance and procedures around their dialysis schedule.
- Assist access nurses in delivering specific care, for example, phlebotomy, basic ultrasound skills, assessment of vascular access and use of vascular access surveillance device.
