The post holder will work as a member of the District Nursing Service under the direction and supervision of the Team Leader (District Nursing).

The post holder will contribute to the implementation of nursing care as assessed and prescribed by the Registered Nurse. This will include working in a multi-disciplinary and multi-agency arena alongside the primary health care team. The post holder will perform a variety of tasks for the District Nursing teams in the geographical area.

The post holder will hold Level 3 NVQ/QSF in a health care associated certificate or possess skills, knowledge and experience to equivalent level or be willing to undertake the course.

The post holder will be required to undertake a range of clinical and clerical activities as delegated by the Caseload holder District Nursing. These activities will be carried out in a variety of settings including patients’ homes, clinic settings, GP surgeries, Residential and Nursing Homes.

The post holder will also contribute to the maintenance of medical and surgical stock levels adhering to the agreed minimum and maximum stock levels required for the District Nursing Team.

The Community Health Care Support Worker will be responsible for providing support to the District Nursing Team in the day to day management of multi-disciplinary/multi-agency packages. These packages consist of scheduled and unscheduled care for patients with multiple, complex and palliative care needs. This will ensure that an efficient and effective service is delivered to the practice population.

All patient activities will be delegated by the Caseload holder ,District Nurse/Registered Nurse. The post holder will then be responsible for prioritising these activities according to the patients’ clinical need and geographical location, unless otherwise specified by the delegating Nurse.

The post holder must recognise and work within the limits of his/her competence actively seeking the appropriate skills for self as well as other members of the nursing team.

They will provide nursing care encompassing a variety of clinical interventions to all adult patients including the transitional cohort as delegated by D Caseload holder district Nurse/Registered Nurse. The anticipated outcome in all cases will be a promotion of quality of life and end-of-life care, maximising independence and minimising the effects of debilitating conditions.

The post holder will recognise deterioration and/or increased need of patients, and escalate concerns to the operational lead/ Caseload holder /Registered Nurse as soon as possible to enable prompt intervention and reassessment of their needs as appropriate.

The post holder must be able to identify situations where prioritisation is essential in determining the need for call to the GP or emergency services is required eg. patient in a collapsed state.

They will adopt appropriate verbal, non-verbal, written and electronic communication skill, and demonstrate communication and negotiation skills in situations which will be highly sensitive and emotive. All communication will be underpinned by the principles of data protection, dignity and confidentiality.

The post holder will answer the telephone, record messages accurately and ensure the information is received by the Registered Nurse in a timely manner.

They will be open and transparent and adhere to the standards documented in the Code of Conduct for Health Care Support Workers in NHS Wales (WG 2011).

As delegated by the D Caseload holder district Nurse, the post holder will source patients/relatives/carers views and feedback on their experience of District Nursing Services in order to influence and inform future service provision.

The post holder will attend meetings as required and ensure that information is fed back to the Caseload holder

They will have an awareness of the triggers and process for incident reporting, risk management, safeguarding, concerns and wider governance related issues and escalate to the /Re Caseload holder registered Nurse.

Be able to recognise Safeguarding concerns, and where appropriate, initiate the safeguarding procedure, taking into account the Deprivation of Liberty Safeguards and Mental Capacity Act requirements.
