**SUMMARY OF MAIN DUTIES**:
The Practice is looking to increase the healthcare team and is looking for someone with the following skills - healthcare assistant or phlebotomist. We recognise that Healthcare Assistants have varying levels of experience and we will provide the necessary training and support to further develop your skills if you are interested.

To work as a member of the general practice nursing team under the guidance of a registered nurse, to support the organisation and facilitation of clinical care in a holistic and proactive manner to patients

To maintain a high professional standard of nursing care in adherence to all practice protocols/policies and guidelines, as well as to provide nursing assistance to the GPs and other members of the primary healthcare team

This role holder needs to be flexible in line with the present atmosphere for change within the health service delivery. It is essential that this role holder works within their own level of competency and to expand their role in accordance with practice requirements and own scope of practice.

The practice has a number of patients who are housebound or live in care homes and the post holder may be required to visit these patients in the community to obtain blood samples or to take physiological measurements.

**DUTIES & RESPONSIBILITIES**:
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Enable patients, families and carers to develop their knowledge and understanding about their health and well-being and be as self-managing as possible

Demonstrate a range of technical and clinical skills and competencies in accordance with the needs of the service

Demonstrate an ability to work under pressure, prioritising workload effectively

Demonstrate an ability to use a problem-solving approach within limits of competence

Demonstrate an ability to provide holistic patient-centred care

Undertake, record and follow guidelines for tasks for which you have received appropriate training and achieved competency

Offer health promotion programmes by providing a holistic assessment, which may include:

- BP checks, height, weight, BMI, urinalysis and smoking status
- Breast health awareness
- Cervical screening
- New patient checks
- Well man/woman checks
- Advice for healthy eating, exercise, alcohol consumption and smoking

Perform routine nursing tasks such as:

- Venepuncture
- Recording ECG
- Chaperoning and assisting patients
- Dressings and wound care

Offer advice about childhood and adult vaccinations and administer vaccines according to current guidelines and legislation

Administer and monitor medication consistent with protocols, standards, and legislation

Monitor and manage maintenance of stock and equipment, reporting to nurses / PCN Manager as required

Perform other clinical tasks as trained and agreed to meet the needs of the Practice. This may include:

- Chronic disease management reviews including asthma, diabetic, non-diabetic hyperglycaemia cardiovascular disease, high risk of cardiovascular disease and hypertension
- Consultations for diabetics including lifestyle advice, BP, weight, height, foot assessments and referring to suitable registered practitioners

Perform wound care following a documented plan of care completed by a suitable qualified practitioner

Prepare specimens for investigation by the pathology laboratory

Assist in the provision of minor surgery and surgical procedures assist in raising awareness and promotion of health within own competence

Give accurate and appropriate information to patients within own competence

Assist the patient and other clinicians in the delivery of care in urgent or emergency situations, including initiation of effective emergency care

Act as chaperone and assist patients

Prepare and maintain environments and equipment before, during and after patient care interventions according to Infection Control guidelines

Undertake infection control monitoring

Assist nurses with the maintenance of stocks of vaccines and other drugs in according to current guidelines and practice policy

Maintain and control stock levels of clinical equipment and supplies, reporting to the senior nurse as required

Physically assist in the transfer and movement of materials and equipment

Maintain accurate and complete records and other documentation in accordance with Practice protocol

Be aware of and recognised issues relating to safeguarding for both adult and child and reports any concerns to the appropriate person

Work with the practice population and wider community to promote healthy living and encourage uptake of services.

**Job Type**: Part-time

Pay: £25,000.00-£27,000.00 per year

Expected hours: 20 - 30 per week
