Advanced clinical practitioners required for the Wrexham and Flintshire urgent primary care centres in the East of BCUHB located in Wrexham Maelor Hospital and Mold Community Hospital, are now recruiting staff both from the nursing and other HCPC members with a full Msc in advanced clinical practice and able to prescribe with significant experience in primary care. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Advanced clinical practitioners required for the Wrexham/ Flintshire urgent primary care centres in the East of BCUHB located in Wrexham Maelor/ Mold Community Hospital, now recruiting staff from the nursing and HCPC members with a full Msc in advanced clinical practice and able to prescribe with significant experience in primary care. As an advanced clinical practitioner you will be working as a member of the urgent primary care team to provide care to patients referred from primary care, emergency department, minor injuries, and community pharmacy with acute onset, single issue, urgent care needs. You will be assessing patients’ clinical needs and providing safe and effective patient care delivered utilising clinical skills, incorporating history-taking, physical assessment, diagnosis and liaising where appropriate with other healthcare professionals. Central to the role will be the use of critical thinking and advanced decision making skills in order to accurately assess and manage patients with a wide range of clinical conditions/illnesses, this may be over the phone (or via other forms of media such as video consultation) or face to face in the urgent primary care centres.
