The Transition Support Worker will be a vital member of a multidisciplinary team within the AMHS service, focusing on supporting individuals following discharge from acute mental health care. This role targets patients with frequent admissions, aiming to minimise the risk of future readmissions through tailored, person-centred support.
In this capacity, you will facilitate a smooth transition back into the community by providing cohesive, person-centred care that ensures continuity of support. You will assist patients identified by the Clinical Lead and Transition team, offering both practical and emotional support to help them achieve greater independence. Your approach will be solution-focused, prioritising the specific needs and preferences of the individuals you support.
