The Teenage Cancer Trust Outreach Youth Support Coordinator is a core member of the Teenage & Young Adult (TYA) multidisciplinary team, responsible for providing both individually tailored and group support to 16-24 year olds. The post holder will be based on the Teenage Cancer Trust Unit at the Royal Marsden Sutton site one day per week but will be providing support to young people throughout the South Thames region, both within and outside of the hospital environment.

The Outreach Youth support coordinator will work collaboratively with clinicians and service managers to both lead and contribute to a wide range of appropriate recreational activities to enable and encourage patients to socialize and remain active as young people, both through the cancer trajectory and transitioning to live with and after cancer.

**Teenage & Young Adult team**:

- The Royal Marsden NHS Foundation Trust is the Principal Treatment centre (PTC) for the South Thames region providing treatment and support for 16-24-year-olds within the region
- The TYA team across the region includes the team based at the PTC and an outreach model of TYA clinical nurse specialists (CNS) based at the 9 designated hospitals within the region with overarching support and accountability provided by the TYA lead nurse at the PTC and the various TYA medical leads.
- The TYA team recognise that TYAs are a heterogenous group with unique psychosocial, information and educational needs, facing the often-competing challenges of adapting to life with and after cancer, and the transition to adulthood. The team aims to provide specialist care and emotional support throughout this time to support the young person through both their treatment, to living with and beyond cancer
- The emphasis throughout and guiding principal of the TYA team is to put the young person first and their cancer diagnosis second
- The team provides support to all our TYA patients accessing treatment throughout the South Thames region and consists of a TYA lead clinician and TYA lead nurse based at the PTC, with two dedicated TYA advanced nurse practitioners and the youth support coordinator also based at the PTC. Young people accessing treatment through their local designated hospital are supported by their local TYA CNS with additional support provided by the TYA team at the PTC. These individuals are supported by allied health professionals including physiotherapists, occupational therapists, dieticians, psychologists and Young Lives Versus Cancer, and partnership working with third sector agencies
